Question 

When a patient is transferring facilities or units it is important to have a medication reconciliation to ensure that you are administering medications based on the most recent physician order. This reconciliation occurs upon admission, when transferring clients and at discharge to make sure that administration is occurring accurately (ATI, 2023). Throughout the coursework I have also seen the importance of utilizing SBAR (Situation, Background, Assessment and Recommendations), to gain information regarding the patient and communicate changes including medication and dosage changes to the nurse that will be handling the patient after you.
