When the nurse arrives, the client is having diffculty walking upright, reports "the worst headache rve ever had," and states he has trouble seeing out ofhis left eye. He states the problems started just after he woke up at 7:00 a.m. (0700). The client took 2 acetaminophen tablets to ease the pain at 7:30 a.m. (0730), but they did not have any effect. The nurse performs a head-to-toe assessment and documents the findings. Selected client assessment findings are presented in the table below.
