Mrs. J, a 65-year-old female, was admitted to the medical ward with a diagnosis of hypertension and an ischaemic stroke that affected her right side. She has a history of uncontrolled hypertension and type 2 diabetes. On admission, she is alert and orientated but having right sided weakness with mild expressive aphasia. She also looks anxious and easily irritated. Currently, she is on antihypertensive medication and an insulin therapy. Nasogastric tube was inserted for feeding as she is also experiencing dysphagia.
