Use this write up for the patient The client was seen today for medication management related to poor attention and iritabdlity She is self-referred and initiated services due to increasing challenges with irnitability and difficulty focusing The chent reports experiencing high levels of irritability, feelings of being easily overwhelmed, occasional poor sleep, and ruminative thoughts centered around her perceived failures as a mother and frustration over incomplete tasks During the intake, she was occasionally tearful The client has a history of ADHD, specifically inattentive presentation, characterized by impulsivity, lack of attention to detail, poor focus, frequent distractions, misplacing items, and forgetting to complete or follow through with tasks. Notably, there is no indication of hyperactivity. The client disclosed a traumatic history, including physical and emotional abuse during childhood and sexual assault by a family member at age 16. She reported that her father is unknown, and her mother lost custody of her when she was 3 years old, likely due to substance abuse. She was raised by a family friend and lived next door to her mother, both of whom were physically and emotionally abusive.
