REPHRASE C - I'm concerned that we're letting our patient go home. Although the medical record has been checked and finalized and assessed, the patient meets the criteria for discharge, his blood pressure and pulse are substantially elevated prior to discharge. U - I'm afraid he might be headed for another myocardial infarction, and I'm uncomfortable about letting him leave. I think it could be dangerous and may cause further complication to the patient. S - This is a safety issue. I'm uncomfortable for him to leave because there might be another myocardial infarction that may occur and it would be unsafe for the patient if this will happen at home and no immediate action that will be done. I'm asking to re evaluate the patient's situation more closely before deciding to discharge the patient.
