HD is a 24-year-old female with a past medical history of hypertension, GERD, obesity, and opioid use disorder, presenting to the clinic for a post-hospital follow up. HD was previously admitted for a femur fracture and was found to be in alcohol withdrawal. While in the hospital, HD was seen by psychiatry for an evaluation. During the psychiatry evaluation, she was diagnosed with opioid use disorder and found to have 4 of the DSM-5 criteria for alcohol use disorder. Her last alcoholic drink was prior to admission, and last use of opioids was 5 days ago. After being treated for alcohol withdrawal and maintaining abstinence from alcohol for several days, she was discharged home with a referral to this clinic. She admits that she is extremely afraid of needles and is willing to take daily medication if necessary. Vitals BP: 118/74 mmHg Pulse: 67 bpm Temp: 98.2F SpO2: 99% Weight: 312 lbs Height: 5' 4" Allergies: NKDA Meds Lisinopril PO 5 mg daily Pantoprazole PO 40 mg daily Labs: AST 28 ALT 32 CrCl 128 mL/min.
