Question

A 66-year-old patient with past medical history of hypertension, hyperlipidemia and overweight/obesity presents to clinic complaining of right upper quadrant abdominal pain for about a week. The patient states that the pain is constantly there but is worse after eating, and they think it improves with laying supine. Patient rates it a 5/10 currently and says it sometimes goes up to 7-8/10 after eating. Patient is also experiencing nausea and intermittent sweating. Patient VS are 100.8 F, HR 96, RR 18, BP 104/64. Patient is in mild distress, alert and oriented x3 with steady gait when walking to the exam table. Chest is clear to auscultation bilaterally throughout. Cardiac rate and rhythm are regular, S1/S2 auscultated, no S3/S4, no murmurs. Radial, DP, PT pulses are palpable. No edema is noted in the extremities. Skin is intact with no rashes or lesions noted. There is moderate tenderness to palpation of the right upper quadrant. The rest of the abdomen is non-tender. Abdomen is non-distended, and patient has normoactive bowel sounds throughout all 4 quadrants.
