A 40 year old female presents to your clinic with complaints of dark colored urine. Past medical history includes obesity and hypothyroidism. She has allergy to bactrim (reaction hives). Past surgical history includes a c-section. She takes levothyroxine and has a mirena iud. She denies dysuria and is in a monogomous relationship with her husband of 20 years. She tells you she is not spotting and does not have periods since the Mirena was placed. She reports having COVID 1 month ago. It was an uneventful course which lasted about 5 days. When she returned to work she was urged to get the COVID vaccination. She had a strong reaction to the vaccine which included muscle aches, joint pain, cola colored urine, and extreme fatigue for four days. On day 5 she is present for an evaluation and is concerned about her kidneys.
