Question

Starting in January of 2021, providers were able to select the level of office visit using either medical decisionmaking alone or total time (excluding staff time) on the date of service (Self, et al., 2020). After reviewing thenew patient SOAP note for Liam, the Medical Decision-Making (MDM) model was chosen to determine thelevel of billing. Providers must be aware the medical decision making is made up of 3 factors: problemsaddressed, data reviewed, and the patient's risk. Although not used to determine the level of billing, Liam'sSOAP note also contains an extended HPI surrounding psychiatric symptoms. These elements includeddepressed moods, isolative and avoidance behaviors, social anxiety with physiologic symptoms, increased stressrelated to his academic performance, frequent rumination, and poor concentration. An extensive physicalexamination was performed on 12 different systems.
