Mrs. Johns is an 86-year-old woman living alone and managing multiple chronic health conditions, including hypertension, osteoarthritis, COPD, glaucoma, and macular degeneration. She self-administers her medications but has recently run out of her antihypertensive medication and has not refilled it and has not been taking her medications as prescribed. Mrs. Johns has a supportive network, including a daughter who visits weekly and a son and friends with whom she has daily phone contact. Mrs. Johns says that she is fine managing her own care but her forgetfulness in refilling her prescription indicates a need for better medication management and possibly more frequent assistance. She spends most of her time lying down and watching TV and not been taking care of her hygiene needs.
