He has recently become wheelchair-bound and lost his mother. His current diagnoses include: congestive heart failure, type II diabetes, and renal insufficiency. For each of these conditions, Arnold sees a different provider from three different healthcare institutions-yet all are managed under the same large medical corporation. Arnold takes eight prescriptions and utilizes the services of three different pharmacies. He previously did not need to keep extensive health records but is becoming increasingly frustrated with the lack of communication among his providers.
