How and when will you monitor or review the plan?
You currently provide support for Pam, 2 mornings and 2 afternoons a week. Pam is a 48 year-old woman who lives in a unit independently and has been recently been diagnosed with Motor Neurone Disease.
Pam is a highly educated woman who has a business background and is described by those who know her as a high achiever with a previous strong work ethic. Pam previously had a large network of friends and close family ties with her two children (aged in their early twenties).
Since Pam's diagnoses of Motor Neurone Disease, her social drinking has turned into a daily habit that is having huge impact on her life. Pam was recently banned from the local pub due to an incident where bar staff refused to serve Pam drinks due to her drunken state, Pam became aggressive and threw an empty glass at the publican's head. Pam has also had fights with both her children after they approached her about her increasingly heavy drinking. Pam now refuses to have anything to do with her children and has slowly isolated herself from most of her friends.
As Pam's support worker you are increasing concerned about her mental health, her physical health needs, her changing environmental & mobility needs and her social isolation. You ask Pam if you can support her in making some plans for the future and to support her in coordinating other supports. Keep in mind your scope of practice as you answer the following questions.
