Please assist with the step by step process and how to properly illustrate the following and provide illustration for better visual understanding.
Part I Data Relationship Model
Use the Microsoft Access Database Tools to make a Relationship diagram showing tables VisitReason and Payer and the relationship between them.

Part II Visit Reason Report
Use the Report Wizard to make a simple report showing these fields from the VisitReason table in this order: VisitPayerID, VisitReason, VisitDate,. The report should be grouped by VisitReason with a Layout type of Blocked.

Part III Payer Mix Report
Payer mix refers to the payer entities that reimburse a facility for services. Examples of payers include Medicare, Medicaid, and Managed care, self-pay and Private Insurance (provided through employer-sponsored benefits plans like BCBS and Aetna). Each payer has a different set of payment characteristics. Facilities spend a lot of time managing the payer mix in order to increase reimbursement by reducing the time between billing and payment.
Make a Report with Fields in this order: PayerClass, PayerCharges, PercentTotCharges, PercentTotPayments, PercentAR and VisitReason.
Be sure to select View data by Payer in the Report Wizard.
To get full credit you may wish to use the design view and resize the labels so that all the text for the report headers is visible (e.g PercentTotCharges, etc.).
