She presents to the pediatrician's office with complaints of increased wheezing, coughing, and shortness of breath over the past week. Her symptoms have been exacerbated by exposure to cold air and exercise. Sarah's asthma is usually well-controlled with a low-dose inhaled corticosteroid (ICS) as maintenance therapy, but she has been experiencing more frequent exacerbations lately. Medication History: Maintenance Therapy: Sarah has been prescribed a low-dose inhaled corticosteroid (ICS) as maintenance therapy to control her asthma symptoms. She typically uses a metered-dose inhaler (MDI) with a spacer device twice daily. Rescue Medication: Sarah also has a short-acting beta agonist (SABA) inhaler for symptom relief during asthma exacerbations. She uses the SABA inhaler as needed for acute bronchodilation. Clinical Assessment: Physical Examination: Sarah appears anxious and is using accessory muscles for breathing. Auscultation reveals wheezing bilaterally. Spirometry: Spirometry demonstrates reversible airflow obstruction with a significant improvement in forced expiratory volume in 1 second (FEV1) after administration of a bronchodilator. Peak Expiratory Flow (PEF): Sarah's PEF measurements indicate airflow limitation and variability.
