Question

The patient's S1 is loudest at the apex, S2 loudest at the base of the heart. No murmurs were noted. Carotid arteries auscultated bilaterally with the bell of the stethoscope, and no bruits were noted. No thrill or Heave was noted upon palpation of carotid arteries individually. Aortic, pulmonic, Erb's, tricuspid, and mitral areas of heart auscultated with diaphragm and bell of stethoscope with no clicks, rubs, or murmurs noted. Head of bed lowered to supine position and pt. The patient was assisted into a left lateral position. Apical impulse located and pulse palpated. Apical pulse auscultated for 1 minute at mitral site.
