Question

The patient is a 57-year-old white male with a longstanding history of hypertension, coronary artery disease, and chronic obstructive pulmonary disease who was admitted for chest pain. The patient has recurrent pain episodes and is followed by South City Cardiology. The patient had a stress test prior to admission, which did not reveal any ischemia. The patient is ruled out for myocardial infarction and was stable over two nights in the hospital. He also had a CT scan, heart (right and left), negative for dissection. The patient was discharged to home in good condition without any further pain. Discharge services took approximately 25 minutes. He is to follow up with his primary care physician, Dr. Lorrenez, in 1-2 weeks after discharge and with Dr. Macavoy, his cardiologist, 2 weeks after discharge. He is to resume his home medications.
