Phenobarbital is the longest established anticonvulsant medication that is still used commonly to treat certain types of epilepsy. Following long-term treatment with phenobarbital, liver microsomal enzymes that metabolize phenobarbital, warfarin, ibuprofen, cyclosporin and other drugs are 'induced' or up-regulated in terms of their activity. In view of this phenomenon, indicate what changes (increase, decrease or none) you would expect to occur in the following pharmacokinetic parameters for ibuprofen in an epileptic patient that has undergone two weeks of phenobarbital treatment compared to if they took the same dose of ibuprofen before they started taking phenobarbital. You should assume that microsomal enzyme induction has taken place in this person over the course of the two weeks. Please thoroughly explain the reasoning behind your answers:
a. plasma half-life of ibuprofen
b. hepatic clearance of ibuprofen
c. the apparent (calculated) volume of distribution for ibuprofen and the actual volume of distribution for ibuprofen
d. plasma half-life of cyclosporin and warfarin
e. Which specific enzymes are induced by phenobarbital
