A 5-Point Model for Value-Based Healthcare
Develop a SWOT analysis of the CNA of your choice. In your response, please include your assessment (and references) of the following:
Assess the quality of data gathered and used for the needs assessment exercise.
Does the CNA have actionable, realistic goals with accountable parties identified?
Discuss how the CNA will use partnerships to accomplish their goals.
Discuss the overall CNA work plan that was created and describe the CNA report generation process that was engaged by your hospital of choice. Include at least 2 outside references on the CNA process, what you learned about best practices deployed, and/or other relevant professional references regarding the CNA process.
Your report should be in the form of a SWOT analysis. When possible, attach a copy of the original CNA document as an appendix /exhibit with your report.
