Coding Steps:

Question 1. Read the physician documents in each of the inpatient cases and assign the appropriate principal diagnosis code following the UHDDS definition for a principal diagnosis.

Question 2. Read the physician documents in each of the inpatient cases to determine and assign codes to any secondary diagnoses as appropriate following the UHDDS definition for a secondary diagnosis and the ICD-10-CM official coding guidelines.

Question 3. Read the physician documents in each of the inpatient cases to determine if a principal procedure code is applicable, and assign following the UHDDS definition for a principal procedure and the ICD-10- PCS official coding guidelines.

Question 4. Assign applicable secondary procedure codes following the UHDDS definition for a secondary procedure and the ICD-10-PCS official coding guidelines:

- Locate and assign the correct diagnosis codes using the ICD-10-CM coding manual, Volumes 1 and 2.

- Locate and assign the correct procedure codes using the ICD-10-PCS coding manual.

Question 5. Appropriately sequence and assign diagnosis, PO indicator, and procedures codes to the inpatient cases.
