28-year-old G0P0 presents with complaints of amenorrhea.  Last menstrual period was five months ago and feels that her periods have been getting progressively worse in regularity. She also reports having trouble losing weight, and worsening acne on her face. She is sexually active using condoms. No history of abnormal paps or infections.

Medications: Fluoxetine 10mg qd

Social: smokes ½ ppd; social ETOH; no illicit drug use; occasional exercise

Vitals: 5'4" 178 lbs; 120/68; 84
