Problem: Mary was recently hired to be the HIM director for Mt. Vernon Hospital. She is really excited about this opportunity because it will expand her knowledge of electronic records. Mt. Vernon is a 300-bed hospital and has several outpatient clinics. The hospital transitioned to an EHR, but the outpatient clinics' records are still in paper form because the physician and staff in the clinics were resistant to moving to an electronic record. Therefore, in all of the facilities, some of the documentation is electronic and some is not. When patients are discharged from the hospital and referred to one of the outpatient clinics, their records are faxed, delivered, or mailed. The clinics have a backlog of loose filing and are also incurring rising costs for storage for inactive records.
