Dwight Thatcher (D. T.) is extubated by the nurse who has provided his care for several days. When he begins to talk, he exhibits an easy sense of humor and shares with the nurse how much he misses being at home with his wife and how much she likes chocolate. He jokingly wonders how he can "sneak out" to buy her a box of candy.
During medical rounds, the attending physician walks into D. T.'s room and bluntly, yet gently, tells D. T. that he has bronchoalveolar carcinoma; he says that is untreatable in its advanced state and will lead to his death. The physician suggests that D. T. start thinking about whether he wants to be intubated or resuscitated in the event of respiratory distress or cardiac arrest. No family members are present when the physician conveys this information, and D. T. is left to tell his family about his recent diagnosis.
In D. T.'s case, what situational and verbal cues indicate a need for spiritual care? Please report which part of the CJM is being addressed and why.
When the physician leaves, the nurse holds D. T.'s hand and expresses care and concern while also feeling sad in view of his situation. He shares with the nurse that he just wants to go home so that he can die in his own bed. The nurse points out that the doctor did not say that he was going to die right away and reminds D. T. that he has a box of chocolates to buy, which makes him laugh. He starts wondering aloud if he should tell his son or his wife first. He begins to talk about his children and grandchildren whom he will "leave behind." The nurse listens to his stories, providing a compassionate presence, and holds his hand for quite a while because it is evident that he wants company.
