Question

In Case 6-1 the patient is very sick and is believed to have a life threatening illness that needs immediate attention. But he is disoriented and does not appear to be fully competent to make decisions for himself. The procedure the doctors want to perform has a 10 percent non-survival rate. The patient appears to resist the suggestion of surgery needed to save his life but it is uncertain whether he is competent or not because of his dementia. His wife is available and she could act as a surrogate. But it is still unclear whether she can make the decision for him. Can she say "yes" to the procedure when he seems to be saying "no." Just how autonomous is he? What do you think the physicians should do in this case? What moral theories or principles would you appeal to?
