The nurse is caring for the patient on postoperative day one after laparoscopic gastric bypass surgery. The patient is consistently rating his pain as 9/10 despite trying non-pharmacologic and ordered pharmacological interventions. The patient is also complaining of extreme nausea and refusing to ambulate as ordered. Ordered pain medication and anti-nausea were administered one hour ago with minimal relief of symptoms. Abdominal assessment reveals that laparoscopic sites are clean and dry, abdomen is obese, soft, and slightly tender with hypoactive bowel sounds. The medications ordered are as follows: hydrocodone-acetaminophen 7.5mg/325 mg in 15 mL solution every 4 hours as needed for pain; ondansetron 4 mg tab SL every six hours as needed for nausea. 8.
