When assessing his pupils, his right pupil is 3 mm, round, reacts to light briskly and accommodates. His left pupil is 4 mm, round, and reacts to light sluggishly. When assessing neuro status, he opens his eyes to pain, withdraws from painful stimuli, and has disoriented speech. How should the nurse document this client's mental status?
