Case Study - Keith's Story

Keith is a 48-year-old male with a 10 year history of ulcerative colitis. He is an active fitness instructor at the Good Life Gym and enjoys the social aspects of hanging out with his fitness friends. After multiple unsuccessful attempted treatments with many medication regimens, including biologics, Keith was recommended for health and quality of life to undergo a surgical colectomy. This procedure required the placement of a permanent ileostomy. Keith arrived on your surgical unit 4 days ago following his surgery. His postoperative course of recovery has been uneventful and there is a plan for discharge tomorrow. You have set aside time today to do discharge teaching with Keith and I would like to frame your discussion using the Roy Adaptation Model. As you prepare for your teaching session with Keith, he asks you to make sure that his partner, Derek, is not visiting during the session because he doesn't want Derek to have to see his ileostomy (ever) or hear all the "gory details". Half-jokingly, Keith says to you, "yup, well, I guess I've kinda given up on the idea of sex...forever".

You are making notes for yourself that relate to Roy's four modes of adaptation and how they apply to the journey Keith is on. While it will be essential to speak with Keith about what he perceives his learning needs to be, there are also some broad elements framed within the RAM and particularly the 4 Adaptive Modes that you believe will be a comprehensive approach.
