Jane is sorting through the pile of health records that she has to process before she can file them in their rightful place. In the middle of the pile, she finds a file folder that is riddled with coffee stains that cover the information about who checked out the file last and when. When she opens the file, she sees that Dr. Green was the last health professional to take notes in the record-six months earlier! Dr. Green didn't authenticate his last entry, which related to a patient who had been admitted for a complex heart surgery, or complete a discharge summary.
