Question: Do you believe individual and community intervention by a community health worker can truly affect social determinants of health, or is upper-level systemic change what is most effective? Explain why, or why not.

Without taking away from the importance of an individual and community intervention by a community health worker is vital to having an effect on social determinants of health. I believe it takes all parties to be the most effective change. I look at it as it is divided into a different tier. Each tier serves a purpose solely, yet there would be a breakdown if all tiers did not work together. The CHWs represent the community, so there is great importance of the individual and community intervention. The community can voice their concerns and the CHW provides the services to improve health. The upper-level systematic changes are also required to affect social determinants of health. The CHWs "help to increase the two-way flow of information so that both providers and those they serve can identify ways to best support health gains and use formal medical and social prevention and treatment services" (Berthold, 2016).
