Part I: Review your own health insurance, answering the following questions. If you do not have health insurance, or do not wish to examine your own policy, select a plan available through the Marketplace to describe. The purpose of this review is to learn more about individual health insurance. The report does not need to be in a narrative form however, be sure to answer all of the questions. You may list the pertinent information (Please be sure to provide a reference citing where you obtained information regarding your health insurance plan).

1. Is this policy an HMO or other form of managed care? Are there any physician or hospital restrictions?
2. Is permission of the insurer necessary for hospitalization? Is there a penalty if the insurer is not notified?
3. Who is covered? What is the monthly premium? Include any amount of premium paid by an employer. In order to determine the amount paid the employer, you may need to call your Benefits Manager.
4. Is a deductible required? Does it apply only to certain services? If this is a family policy, does the policy look at individual deductibles or is there a family deductible?
5. Is there a copayment required? For all services?
6. If there are copayments and/or deductibles in the policy, are there any services to which the deductible and/or copay do not apply?
7. Was any special coverage purchased?
8. Are there any exclusions? List several of them.
9. What type of coverage is available prescription drugs?
10. How has your current plan changed, if any, to meet the requirements of the PPACA?

Part II: As you review your own health insurance policy for the second part of the written assignment, think about how health insurance status influences health care access and utilization in the United States. With this in mind, describe one way insurance coverage has enhanced utilization of health care services and one way health insurance coverage has impeded utilization of health care services in the US. As you respond to the second part of the assignment, consider the recent changes to the ACA that will influence health insurance status in the United States. Be sure you support your response with evidence from the literature to substantiate your opinion.
