Situation: You are assigned Mrs. Aster Bellows. She is a 56-year-old woman in the Emergency room transferred in from another hospital via an ambulance. She presents with symptoms of shortness of breath (SOB).
Background: Mrs. Bellows is a housewife. Her Body Mass Index value of 26.0 kg/m2 based on her height of 1.58 m and weight of 65 kg indicated that she fell into the overweight range. Based on her past medical history, the patient was diagnosed with pulmonary tuberculosis for the past 3 months, hypertension and diabetes for the past 5 years and advanced renal failure for the past 6 months. She is a non-smoker and a non-drinker, and lives with her daughter, Ambyr.
Assessment: She presents with symptoms of shortness of breath (SOB) and her respiratory rate is 24 breaths per minute. She appears pale and weak, and her blood glucose level is low at 60 mEq/L and her blood pressure is 152/93 mmHg, pulse 110, & temperature is 100.2' F (37.9' C). Her lungs are diminished throughout. Lung expansion is equal bilaterally. Her abdomen is soft and non-distended with normoactive bowel sounds. Several diagnostic tests were performed to evaluate the patient's condition. A positive sputum smear test indicated that the patient's tuberculosis was still active. A chest X ray was conducted on this patient and shows minor lesions at the apical segments of the upper lobe.
