At this point, Carlos has been admitted to the hospital for observation and further lab testing in hopes of getting a better overall picture of his condition. Two hours after admission, the team gets the results of one of the blood tests. The results indicate that Carlos has anemia (a low red blood cell count) and thrombocytopenia (a low platelet count). These are uncommon findings with gastroenteritis, and point to something more serious going on in Carlos' body. What could be causing his symptoms?

Carlos' body needs to recapture the water in the nephron filtrate and return it to the blood plasma before it is lost in the urine. The medullary collecting system is the last chance to adjust the composition of filtrate before it leaves the kidney as urine. All of the following might happen in Carlos' medullary collecting system except:

All of the following might happen in Carlos' medullary collecting system except:

a. Hydrogen ions are actively secreted into the filtrate.

b. Some urea moves into the interstitial fluid from the filtrate.

c. Antidiuretic hormone (ADH) will cause the cells of the medullary collecting system to become impermeable to water.
