Scenario: Mr. Amos Miller Age: 71 Admitted: Two weeks ago after several seizures Diagnosed with: Epilepsy (first diagnosed 10 years ago), Hypertension, and Type 2 Diabetes Mr. Miller has a history of controlled epilepsy with medication (Carbamazepine), but recently, he experienced multiple breakthrough seizures due to missed doses of his anti-seizure medication. During his hospital stay, his medication was adjusted, and his seizures have been controlled. In addition to epilepsy, Mr. Miller has long-standing hypertension and Type 2 Diabetes, both of which are managed with medications. Current status Mr. Miller is now seizure-free for 7 days. His blood pressure and blood sugar levels have been stable with medication adjustments. He is ambulatory but needs assistance with ADLs (Activities of Daily Living) due to weakness after the seizures. He will need ongoing support with medication adherence, particularly with his anti-seizure, blood pressure, and diabetes medications. Discharge Plan: 1.
