Evidence-based screening tools are an integral component of mental health and behavioral assessments. Such tools are beneficial in practice to quickly and accurately diagnose. Therefore, it is essential to understand the different screening tools available and the validity and reliability of each tool to determine the appropriate tool for each patient. First, individually identify a valid and reliable screening tool with a focus on behavioral and mental health that helps a Family Nurse Practitioner (FNP) in the assessment.

1. Provide a description and/or link to the screening tool you found and critique the screening tool to determine its value for a primary care setting, including its ease of use, scoring and interpretation, and limitations.

2. Discuss a patient example that would fit the need for such a screening tool and discuss additional physical exam components, clinical management plan, and/or referrals the FNP would need to do based on the knowledge gained from the screening tool.

3. Identify key referrals and interprofessional team members that would be appropriate for managing the patients based upon screening tool results, with rationale.
