Question

Walter is a 42-year-old male admitted per court order to the inpatient mental health unit. Walter was picked up by police after he was found wandering in the snow without shoes and wearing only shorts and a T-shirt in frigid weather. Walter has frostbite on his feet and hands. He is disheveled, unshaven, paranoid, and agitated. He was very guarded and reluctant to talk on admission. Walter's brother Michael provided information to the nurse. Walter is a self-employed contractor, who works long hours and has had a very busy year. Michael states that Walter has been so busy that he has had to work through the nights with just a few hours of sleep for the past few months. Michael states, "I don't know how he has managed; he is ruining his health and has been irritable and acting strangely the last few times I have seen him. He doesn't even look like the same person." Michael states that Walter has no history of mental illness or substance abuse. Walter's toxicology screen was positive for methamphetamine. Walter was given an antipsychotic, and a benzodiazepine in the emergency room prior to admission for delusions and agitation.
