Patient is a, seen today for an initial outpatient psychiatry evaluation. He reports past medical history of HTN on losartin 50 mg QD. Appendectomy in 2023. Reports allergies to PCN.Has PCP at JHH. He has been dx with MDD on therapy. He has been on prozac, effexor in the past. Now on wellbutrin 450 mg daily.He reports he has been suffering from anxiety and panic attacks.Has been tried on Propranolol, hydroxyzine, gabapentine, trazodone and all of them do not work.He reports he was adviced by his provider that he should seek another opinion elsewhere as all possible treatment had been exhausted and they could not prescribe for him advanced treatments. He further reports severe lack of attention and concentration and thinks he has ADHD.He attributes lack of medical treatment to his Chineese parents who he described as not believing in conditions such as ADHD. He denies childhood traumas.
