Patient is a 29 year old white lady seen today for an initial outpatient psychiatry evaluation. She denies medical/surgical issues. She denies drug allergies. She reports at age 19 years old she was diagnosed with ADHD. She reports she took adderall 20 mg xr THREE DAYS AGO.She reports she gets her prescription from her PCP in Pennsylvania. Her mom picks it up for her and sends it to her. She is in the process of switching doctors.About a year ago she was diagnosed with anxiety and depression and she was put on medications that she did not like. She last took this medications a month ago.
