He presents with complains of wanting to establish mental health care provider for continuity of his care.HHe reports past medical history of asthma, seizures. Reports history of plates on his jaws, TBI due to motorbike accident in 2020 status post cardiac arrest and tracheostomy.He denies any known drug allergies but his discharge summwrry indicates he has allergies to escitalopram, haloperidol, milk containing products and onions.Reports helplessness, hopelessness.
