Reports history of hernia repair and tonsilectomy as a child. Reports allergy to keflex. He reports he was diagnosed in the 1990Swith PTSD, ANXIETY, panic attacks AND DEPRESSION. He reports he has history of being kidnapped perpetuated by his exgirlfriend. He is on pristiq, buspar and vrayler.He is unsure of his dosages. He reports ongoing anxiety.He reports he last took his medications 3 days ago.He reports he has been tried on multiple medications.
