Patient is a 40 year old white female, seen today for an initial outpatient psychiatry evaluation. She reports surgical history of right ankle and left heel.She reports she does not have a PCP AND DENIES ANY KNOWN DRUG ALLERGIES. She reports she has been diagnosed with GAD, panic attacks, bipolar (2005), ADHD (2000) AND ptsd.She reports history of associated at bayview, Franklin square and sheppard Pratt hospitals.She reports she has been on multiple psychotropics including zoloft, prozac, seroquel, clonodine, xanax, welbutrin, klonopin and risperdal.She reports she stopped taking her medications months ago and she is here to restart her treatment. She reports she is on methadone 70 mg QD and she is planning on cutting down. The patient reports when she is a maniac, she feels a lot of energy and her mood is elevated.Reports easy distractibility. Becomes more irritable and very talkative. Reports racing thoughts.
