Question

You are employed as an enrolled nurse (EN) at the Indigo Community Services and Health Hub (ICSHH), providing care to clients as part of a multidisciplinary care team. Alex Romano, 75 years, is an existing client who lives 50 kms from Goodooga, a remote area of NSW. Alex has limited English language skills that he learnt after arriving in Australia from Italy in the 1980s. Alex visited the clinic today because he is experiencing polyuria, polydipsia, polyphagia and weight loss. After consulting with the Registered Nurse, you check Alex's vital signs and find they are normal. You calculate his BMI to be 29.1, then complete a BGL and urinalysis. A Medical Officer (MO) examines Alex and reviews the preliminary test results, orders a full set of bloods and discusses with Alex the possible diagnosis of Type 2 Diabetes Mellitus. The MO recommends that Alex complete BGL tests 6 times each day and has requested that Alex visit the clinic twice a week and have home nursing on alternate days.
