68-year-old man is brought in by his wife and son for increasing mood swings and forgetfulness. His problems began several years ago when he began having emotional outbursts at work, which is very uncharacteristic for him. He would tell his employees to finish projects and then forget that he asked; later he would get upset why other projects that he never requested were not completed. This began to happen more often and although he was embarrassed about his angry outbursts, he would contend that his recollection was correct and his outbursts justified. He has similar incidents at home and his wife admits that she is afraid of him during his outbursts, though he has not harmed her. He has hypertension, coronary artery disease, and long standing smoking habit though he quit six years ago. His MRI scan shows cortical atrophy and widened sulci.
