Question
The RN sees the patient in the hospital ER.
ID: Chenoa (White Deer) is age 19, Menominee Native woman from the Menominee Reservation near Green Bay, Wisconsin
Police: She is Native American from the Menominee Reservation. She was brought in by the police after a shootout in Chippewa Falls, Wisconsin for visible bruises.
Social Worker: Patient had been age 14 when she had been raped by a Caucasian married man who used to get drunk & repeatedly visited her Menomonie Reservation in upstate Wisconsin to abuse her. Her tribe's judicial system had no jurisdiction to indict him, which has to be done in Wisconsin State courts, and her brothers were afraid of confronting him, due to being afraid of Wisconsin jails. Age 19, she became pregnant from this man, so she goes to the nearest town to find out about benefits she might be able to get for her soon to be born baby. He sees her in town & forces her to go with him to his woodland cabin. She was only rescued when the nearby sheriff of Eau Claire Wisconsin took a posse to capture him. Her mother & brothers finally reported that she was missing from the Menominee Reservation to the Wisconsin State court.
VS: RR 27, T 97, HR 90, BP 135/90.
Lab highlights: blood tests, LFTs, STD tests, HBV test, QuantiFERON-TB Gold Plus test
PE highlights: BMI 27, diaphoresis, mydriasis, bruising on left & right dorsal forearms
Gyne/OB: Pregnant, fundus 25 cm,
Psych highlights: attitude: cooperative, eye contact: often absent staring, speech: soft, stunned. thought logical but slowed. Flashbacks, insomnia, detachment, rule out dissociation.
Diagnoses: Vaginal trauma, PTSD, R/O: Derealization.
