The cultural diverse individual is less likely to be compliant, seek or follow medical advice if they are unable to understand or communicate effectively with the healthcare provider. With constant and continuous innovations and regulations regarding awareness of cultural competency the health care system can decrease and eliminate health disparities and inequalities. incorrectly which could result in a quality of care matter just because an interpreter or document translation was not offered. If there is found to be a violation the organization or provider could face fines, penalties, suspension of Federal rebate or loss of licensure. Take for instance a Spanish speaking individual has just prescribed a new high blood medication, asthma, and diabetes medication if the instructions, dosage, and side effects are not explained or given to the individual in their native language this situation could pose a life-threatening condition. The individual could misinterpret the instructions and take the medication incorrectly which could result in a quality of care matter just because an interpreter or document translation was not offered.

These publicly funded health care programs along with government assistance have devoted, developed and initiated different entities to research, analyze and come up with innovations, outreach programs, regulations, education, training, and policies to address these issues of health disparities and inequality (NIMHD, 2018). In my organization every year there are multiple mandatory training for all healthcare staff, managers and frontline, on cultural competency, cultural sensitivity, language assistance, and cross-culture communication. Communication is always a challenge, and when diverse cultures interact, effective communication can be even more challenging. Cultural differences and language barriers have a significant impact on health care compliance on an organization and government level. Federal contractors and Health Care Providers are mandated by CMS, Department and Health and Human Services and other government agencies to be compliant with regulations, policies, and procedures that in line with their goal of equal quality of care for all consumers.

Every consumer in the health care system deserves the same quality and treatment of care, and due to different cultures, beliefs, religions, and customs the health care system is becoming more patient-centered and personalized. Regarding publicly funded health care programs I do not believe that they overrule cultural differences to provide a standard level of care but quite the opposite. Publicly funded health care programs including Center for Medicare and Medicaid Services'' (CMS) primary goal is to provide top quality, affordable and equal health care services to all consumers therefore in 1986 CMS established the Office of Minority Health (OMH), and was reauthorized under the Affordable Care Act in 2010. OMH is a liaison between CMS, government agencies and other publicly funded health care programs who work in collaboration to decrease inequality and health disparities (OMH, 2018).

I believe that within the healthcare program cultural difference and language barrier such as the one experienced by the Lee family have had an impact on the healthcare compliance. One of the key impacts is the use of language interpretation service. There were no Hmong translators at the beginning of Lia''s treatment. It is available now to a patient, language interpretation service should be promoted where community need exist. The use of community health worker such as non-medical personnel who help patients navigate the health care system was one of the recommendations of the committee Congress selected, I believe that will be very effective as well. In addition to multidisciplinary treatment and preventive care teams should be supported. Cultural competent communication at the provider, care institution, health plan, and national level is being introduced but not implemented. Promoting cultural competent communication at these levels can help reduce racial and ethnic disparities in the receipt of high-quality care.

The death rate from cancer, heart disease, and diabetes are significantly higher in racial and ethnic minorities than white these disparities are unacceptable. I was a medical coordinator for a preventive care company we offered five preventive screening that primary physicians didn''t offer that includes carotid artery, abdominal aortic aneurysm and artical fibrillation screening in addition to other screenings majority of my patients/consumers were white. I always wonder why?.. I believe that is because of lack of knowledge meaning as a minority if we understand the problem then we can educated and find solutions but American including healthcare is a system into hard to impose or change in my opinion.

A report that was conducted by the National Medical Association requested by Congress to assess the extent of racial and ethnic differences in the quality of health care received by patients not attribute to known factors such as access to care, ability to pay or insurance coverage; evaluate potential sources of these disparities, including the role of bias , discrimination and stereotyping at the provider, patient, institutional and health system levels. The committee includes 15 members that reviewed all relevant literature gained and further insights from commissioned papers, workshops, focus groups, round-tables discussion and technical liaison panels and information from the public concluded that there are many complex sources of racial and ethnic disparities in healthcare. Key findings show that racial and ethnic disparities in healthcare exist even when insurance status, income, age and severity of conditions are comparable.

Hmong family has a lot of beliefs that would not be understood by other cultures or would think their beliefs creates pressure among there people. Within these health care programs, do cultural differences and language barriers such as the ones experienced by the Lee family have an impact on health care compliance? Cultural differences do impact health care compliance because after she had her baby in chapter 1 and could not read or speak English, no one took the time to explain the paper work and she signed them anyway. These are crucial differences that can save a life in the health care system. If the person who is being treated does not understand, an interpreter should be made available. Not knowing how to comply could have negative affects on patients if they are not careful to understand what is given.

Cultural differences should not deter people from getting the help they need or those who are providing the service especially being in the health care field. People need medical attention every day and providing the type of care needed should be unimpeded. Health care services have state regulations that follow rules and guidelines but, According to “NCBI” (2002), “Many health care providers argue that such regulation adds to their costs, and high- profile problems can create additional tensions that impede collaboration between the state public health agency and the health care delivery system” (The future of the publics health in the 21st century). The cost of cultural differences should not cost anything but learned about different cultures.

The assigned reading about the Lee family very much so goes hand in hand with the subject matter. Despite their strong cultural beliefs regarding what their daughter was experiencing, they still made an effort to make several trips to the emergency room to ensure her safety. Cultural differences and barriers like the ones described in Chapters three and four can potentially have an impact on healthcare compliance given that these compliance standards may be built to protect patient confidentiality, rights, etc. Compliance programs vary among organizations, although the core values are generally similar, there will be detailed differences surrounding the company’s tolerance and policies; this can also depend on the organization’s patient demographic as well. All-in-all, I feel that cultural sensitivity is of a much lower priority for institutions that are publicly funded.

The public forms of payment are known as Medicare and Medicaid and are offered and regulated by the government. Clinics that generally utilize these programs are community health centers and any other establishments able to receive Medicare Reimbursement (Institute of Medicine, 2002).I definitely feel that publicly funded health care programs overrule cultural differences for the sake of providing a standard level of care. I say this because, in order to provide, "Standard" care, they must generalize a common practice to their overall customer base. Privately owned institutions seem to take patient satisfaction and cultural competency much more seriously given that their success and income solely depends on their client satisfaction and business model. Publicly funded programs on the other hand, get paid by the government as they bill for services that they provide on a daily basis. It is important to note that although paid differently, both these sectors must upkeep a certain level of cleanliness, rules and regulations as set forth by the government.
