Question: A 31-year-old Caucasian male named Tex is admitted on an Order of Protective Custody (OPC) to Green Oaks Psychiatric Hospital at Medical City, Dallas. Tex's spouse and high school sweetheart initiated the OPC because Tex was threatening suicide. She is frightened and genuinely concerned about Tex's well-being and safety. Tex has two young children.

Dr. Davis has completed the psychiatric evaluation for Tex and he is now being assessed by a nurse. The nurse notes that the physician orders include placing Tex on suicidal precautions and an order for Haldol 5 mg, & Ativan 2 mg. The nurse notes that Tex seems easily agitated and appears hypervigilant and 'jumpy' in response to noise in the room such as the chair being pulled out or door being shut. The nurse notes that Tex has a tattoo of an American flag and a bald eagle on his right forearm and under it are the letters USMC. The nurse asks, 'Have you served in the military?' Tex answers, "Yes, 10 years and I fought in combat during the Iraqi War." The nurse responds, so you are a veteran then. Tex shakes his head yes, and says, "I have been out for about five months now." He just stares at the floor with no facial expression. The nurse begins a conversation, "Can you tell me what happened during that time? Maybe you have some things that are still upsetting you?" The nurse notes a large tear rolling down his right cheek. The nurse is quiet for a bit and then says, "I know some things are difficult to talk about but it might help." Tex slowly begins to describe a traumatic event he witnessed during combat, but gives little detail. He also said that he is hearing a voice that is making him feel guilty about surviving.

The nurse is aware that many veterans are treated outside of the VA system even when they have benefits to help with care management. The nurse also knows that many persons experiencing Post Traumatic Stress Disorder (PTSD) are not always diagnosed before they are discharged from the military. The nurse recalls that Green Oaks has recently enacted a policy to help health care providers identify veterans with PTSD, an assessment that is critical to quality care for the population. The hospital's new policy requires the health care providers to ask all patients, "Have you ever served in the military? Are you a veteran?" If answered as a positive response, then the nurse is to administer the Primary Care PTSD Screening Tool (PC-PTSD). Tex tests positive on the PC-PTSD scale indicating he may be experiencing PTSD.

The nurse wonders how Green Oaks came to the conclusion to create a policy to administer the PC-PTSD tool as opposed to the PCL-M (or another standard assessment tool) for confirming a PTSD diagnosis. A correct diagnosis is essential for improving health outcomes for the veteran population.
