Joan, a divorced 38-year-old woman, has been diagnosed with a terminal disease, as well as depression. Over the past year, Joan's physical and depression-related symptoms have worsened. One day during a visit to the hospital, Joan announced to her physician and social worker, "I can't stand this pain. Living like this is worse than dying. I'm ready to stop treatment and go into hospice care." Joan has an advanced directive that names her mother, Cindy, as her decision-maker (health care proxy) should she [Joan] become unable to make decisions herself. Cindy disagrees with her daughter's request. She says that her daughter is "too young to die" and views her request as "suicidal." Cindy believes that there are other novel treatments and clinical trials to still try and thinks that Joan's depression is clouding her decision-making. Cindy asks the hospital to deny Joan's request and invokes her power as the decision-maker named.
