Problem
John, a 65-year-old male, was brought to the emergency department after a severe car accident. He was unconscious and had multiple injuries, including a deep laceration on his left thigh and a fractured femur. John has a history of hypertension and type 2 diabetes. His latest lab results showed a hemoglobin level of 6.0 g/dl, significantly lower than the normal range (13.5-17.5 g/dl for men). His vital signs were unstable, with a blood pressure of 90/60 mmHg, heart rate of 110 beats per minute, respiratory rate of 22 breaths per minute, and oxygen saturation of 92% on room air. Given John's low hemoglobin level and unstable vital signs, packed red blood cells (PRBCs) would be the most appropriate choice for John. Identify 3 possible transfusion reactions. Re-create the scenario to incorporate each reaction. Identify special nursing considerations for each scenario, identify treatment considerations for each reaction and determine how your responds to your treatment.
