Case: SITUATION: An audit is admitted with an acute exacerbation of congestive heart failure. Vital signs are : Temperature 37.6 degree centigrade, Pulse Rate of 115/beats per minute, Respiratory Rate of 32/cycles per minute, Blood Pressure 154/100 mmHg. Ankles are edematous and crackles are auscultated at the base of both lungs.
