Assignment: Case Management- Case Plan
Overview:
Now that we are in the assessment and planning phase of case management, it is your turn to practice what you have learned! For this assignment, you will develop a case plan for your client, Sue. This case plan will focus on setting goals and identifying objectives to support your client in achieving each long or short-term goal.
Instructions:
Using the provided Case Plan template, you will develop at least 5 goals and objectives for Sue based on the information you have. Be sure to fill out each column fully for each goal. Here are a few pointers and instructions regarding each column:
1) Type/Need: What type of goal is this? What need does this meet for the client? Is it related to income/finances, housing, education, transportation, medical? Your responses for this column can be one-word responses that identify the type/need.
2) Goal: Identify the specific goal that you and Sue are setting to meet her identified need. Remember to apply the concept of SMART goals to your work.
3) Objectives: Identify at least 3 objectives for each goal. In applying the SMART goal principles, these should be very specific steps that will help Sue achieve the identified goal.
4) Referral: Share what referral you have made for Sue, if needed. For example, if Sue's goal relates to employment, a possible referral may be to a job training program.
5) Follow-Up/Check-In: When will you follow up with Sue? What will you follow-up about: meeting one or more objective? Will you follow up after the goal has been met? Will you follow up with the provider to determine that the referral has been completed?
