Some Engineers of a space shuttle knew about some faulty parts months before takeoff. Bob, a member of the team, stated his concerns to the head engineer, Shane. Shane along with the other members of their tightly knit group of engineers did not want any disagreement among themselves or negative press, as they were focused on launching on the day, they had agreed on months ago. Not wanting to be labeled the odd one or the sore thumb, Bob fell in line with the others and said nothing more about his concerns. Deep within themselves, individual members of the team knew that there could be serious consequences if the launch went ahead without ironing out the glitches, but the general consensus of the group was that the concerns were minor.

The team ignored the faults and concerns raised and pushed along with the launch. The space shuttle had a safe launch and landing, but the astronauts experienced serious in-flight difficulties associated with the faulty parts, it could have been a total catastrophe.
