A home health psychiatric nurse is visiting Sylvia, a 25-year-old female who was diagnosed with schizophrenia 5 years ago. Sylvia lives with her mother who shares her concern that Sylvia has been in her bedroom for the past week, refusing to bathe or brush her teeth. Her hair was uncombed, and she has been wearing the same clothes for the past week. Her mother is concerned as Sylvia is barely eating and not drinking enough fluids. Sylvia believes the FBI is spying on her and is hearing voices that the government is plotting to kill her because she has DNA superpowers. She refuses to eat because she believes the government is trying to poison her and her mother is working with them. Sylvia stopped taking her psychotropic medications, olanzapine PO, a month ago because she was feeling better and did not like the weight gain the medication caused. She missed her last two psychiatric appointments as she refused to leave the house. Her mother finally called her psychiatrist who did a referral for the home health nurse to visit, assess Sylvia, and call with your findings.

As you enter Sylvia's bedroom and introduce yourself, she appears agitated, and states "why you are here, my hair is getting signals from the FBI, I swim, mud, ink, poison, plate me." She walks over to her bed and pulls the cover over her head.
