She missed her last two psychiatric appointments as she refused to leave the house. Her mother finally called her psychiatrist who did a referral for the home health nurse to visit, assess Sylvia, and call with your findings.

As you enter Sylvia's bedroom and introduce yourself, she appears agitated, and states "why you are here, my hair is getting signals from the FBI, I swim, mud, ink, poison, plate me." She walks over to her bed and pulls the cover over her head.
